We provide care coordination and supportive services to clients. We listen to our clients and work to ensure that their needs are addressed holistically. We partner with providers of community resources such as housing, healthcare, treatment, and criminal justice programs to connect clients with the support they need.

Intensive Case Management

Our intensive case management services are for people with high medical or mental health needs. We provide specific services to people with greater vulnerabilities and barriers to care, including people with HIV/AIDS, the elderly, and veterans.

Harm Reduction and Recovery Support Services

For people who use substances like alcohol, heroin, or crack cocaine, current treatment practices do no always meet their needs. We work to bring the full range of services to them – from needle exchange to medication assisted treatment – our work reduces harm and supports healing.

Primary Care

We partner with Neighborcare and Harborview to provide outreach to clients who need medical attention. From diabetes to the flu to wound care, the knowledgeable medical staff can treat clients before symptoms become severe and emergency care is necessary.

Mental Health Services

With Harborview’s Downtown Mental Health Team, we make sure that clients are connected to the mental health services they need to make changes that improve their quality of life.

VITAL – FAMILIAR FACES INITIATIVE

Vital is designed to provide comprehensive and integrated services to adults who are experiencing mental health conditions and co-occurring substance use issues, need an intensive level of community-based support, and may be experiencing homelessness.

A collaboration between REACH, Harborview Medical Center and Plymouth Housing Group, the Vital team employs expertise from diverse disciplines, from psychiatry to occupational therapy to intensive care management. The Vital team provides care that is centered around the participant’s personal goals and aims to improve social and health outcomes, including but not limited to a reduction in jail time and ER visits and an increase in housing stability and social connectedness. Read more about these outcomes.
